Social Prescribers and Care coordinators
Our Social prescribers are here to give people more time, focusing on ‘what matters to you’ and taking more of a holistic approach to people’s Health and Wellbeing. They help patients to connect with community groups and statutory services for practical and emotional support.
There are many support networks across Suffolk and our team would be happy to help you find one that meets your needs. Social prescribing works for a wide range of people, including people:
- With one or more long term condition
- Who need support with their mental health
- Who are lonely or isolated
- Who have complex social needs which affect their wellbeing.
- Who have Housing issues/concerns
- Who have Finance issues/concerns
- Whom may be or has been affected by domestic abuse
- With any NON medical concerns that is causing stress and worry.
